Blood Glucose Monitoring Log Sheet

Why it matters

Checking your blood glucose (blood sugar) levels gives you a snapshot of how your body is responding to food, medications, physical activity, and other factors. How to use the log sheet

  • Before you test: Wash and dry your hands. Follow your meter’s instructions (insert a strip, use a lancet to prick your finger, place the blood drop on the strip).
  • Test at recommended times: Examples include before meals, 1–2 hours after meals, at bedtime, and when you feel different (e.g., shaky, sweaty, very tired). Your healthcare team will tell you how often to test.
  • Log the result: Record the date, time, reading (e.g., mg/dL or mmol/L), what you had just eaten (if this was a post-meal reading), what you were doing (exercise, stressed, etc.), and how you feel (symptoms).
  • Look for patterns: If you notice that your levels are high at the same time every day or after the same meal, bring this to your healthcare team. It might mean your food choices, insulin/medication dose or timing, or activity level need to change.
  • Take the log with you: At clinic visits, a complete log gives your provider much more information than a single meter reading.

Pro tip: Bring the log sheet to every diabetes-care appointment. Use the notes section to jot down things like “ate out at 8 pm”, “walked 30 minutes after dinner”, or “felt unusually tired”. These details help interpret your numbers.

A1C Tracking Chart

Why it matters

The A1C test (also called HbA1c) shows your average blood sugar level over the past ~2 to 3 months. It gives a broader view than daily finger-stick readings. Higher A1C levels are associated with increased risk of complications such as nerve, eye, and kidney damage.

For many adults, the ADA recommends a target A1C of less than 7%, though your personal target may vary depending on age, health status, and risk of low blood sugar.

How to use the tracking chart

  • Record your A1C result each time you have the test. Write down the date and result (for example: “May 2025 → 6.8%”).
  • Convert to estimated average glucose (eAG) if available — this helps you relate the A1C percentage to what your meter shows at home.
  • Track changes over time. If your A1C is going down, it means your overall blood sugar levels over months are improving. If it’s going up or staying above target, it may be time to review your care plan.

If meter readings look good but your A1C is still high, it may mean you have “hidden highs” (e.g., overnight or between testing times) that need attention.

Insulin & Medication Log

For many people with diabetes, taking medications or insulin is a key part of managing blood sugar. According to NIDDK and ADA, keeping track of what you take (which medicine, how much, and when) helps prevent mistakes (such as missing a dose or accidentally doubling it) and gives your healthcare team clear information when adjustments are needed.

In people using insulin, proper timing and dose adjustment are especially important because different types of insulin have different onset, peak, and duration times.

How to use the log

Record the basics:

  • Date & time of medication or insulin dose
  • Name of the medicine or insulin type (e.g., “Insulin glargine 20 units” or “Metformin 500 mg”)
  • Dose taken
  • Reason, if relevant: e.g., “before dinner”, “changed by doctor”, “travel time”
  • Reaction or check result (optional): Did your blood sugar go high or low after this?
  • Track changes: If your doctor or diabetes educator changes the dose or type, note the change and the date.
  • Link with logs: If your blood glucose log shows frequent high readings in the evening, compare with your medication log to see if timing or dose might need review.
  • Bring it to visits: At clinic visits, your physician or diabetes educator will appreciate seeing your medication log — it helps tailor your therapy.

Pro tip: Consider using a simple chart or table with columns like: Date | Time | Medicine/Insulin | Dose | Notes. Keeping it consistent makes it much easier to review. If you switch to a new medication, add a line and highlight the change.

Food & Symptom Diary

Why it matters

What you eat, when you eat, and how you feel after meals all affect your blood glucose. Keeping a food and symptom diary helps you and your healthcare team see which foods or habits may cause your blood sugar to rise or fall, when you feel symptoms, and how other factors (exercise, sleep, stress) influence your numbers. Research shows that tracking these details supports behavior change and diabetes self-management.

How to use the diary

Record meals & snacks:

  • Time of eating
  • What you ate (include major ingredients, sauces, and portion size)
  • Estimated carbohydrate amount (if you count carbs)

Record symptoms or context:

  • How you felt: tired, shaky, sweaty, unusually hungry?
  • Physical activity: did you exercise before or after eating?
  • Stress or illness: headache, sickness, travel, infection?
  • Sleep: were you well rested or short on sleep?

Correlate with glucose readings:

  • After a meal where you logged food, compare your diary with your blood glucose reading 1–2 hours post-meal (or when you normally test). Does the food choice correspond to a higher or lower reading?
  • Over a few days, patterns may appear — for example: “Whenever I have meal X I get a reading >180 mg/dL 1 hour later,” or “Late-night snacks lead to a high fasting reading the next morning.” Bring these patterns to discuss with your team.

Use it for problem-solving: The diary is a tool — not to judge yourself, but to inform changes. If a particular food or meal timing is causing high readings, you and your dietitian or educator can strategize an adjustment.
